Most likely the cause is this KB.

When using the shipping plugin I keep getting this error:

Error: Shipping Rates Cannot Be Determined!
Please provide either a Valid State or Valid Country

I have tried what it says in the knowledge base and what others have tried in this forum and nothing seems to be working.
